The Martha’s Vineyard Community Health Initiative is seeking one-year proposals for up to $5,000 in grant money. A total of $20,000 in mini-grant funding will be awarded to address unmet needs in the areas of behavioral health, public health, chronic disease management and healthcare access. Grant awards must be utilized within one year of receipt of funding.

In order to be selected, proposals must demonstrate a benefit to the Martha’s Vineyard community beyond the life of the project. Preference is given to projects that involve collaboration among Island-based organizations and individuals. Proposals that duplicate existing projects, or ones that lie outside the four specified areas listed above, will be excluded from eligibility.

Applications for the next funding cycle are due by Nov. 1, 2012, with awards announced in December.
